RELEASE OF LIABILITY: In connection with my activities as a volunteer at the 2021 U.S. Amateur Championship (the “Championship”) at Oakmont Country Club and Longue Vue Country Club (club/facility), I agree and understand that my presence at the Championship and any volunteer work or services performed by me for the Championship, its supporters, any attendee of the Championship, the club/facility, USGA, their respective affiliates and subsidiaries and their respective officers, directors, agents, members, employees and officials as well as any other volunteer (collectively, “the Indemnified Parties”), may expose me to both known and unanticipated risks of harm and/or injury. In consideration of and as a prerequisite to my participation as a volunteer, I, for myself and for my heirs, successors and assigns, acknowledge that such risks exist, hereby assume all such risks, and release and discharge the Indemnified Parties and each of them from any and all claims for liability for personal injury (including death) and property damage that I may suffer while performing such volunteer work or service, whether or not on the premises of the club/facility, including, without limitation, any claim arising out of any condition of the premises owned by the facility or owned, leased and/or used by Oakmont, Longue Vue and/or the USGA for, or the conduct of any person in connection with, the preparation for, supervision of or conduct of the Championship or any practice round or activity connected or related to it. I further understand that the Indemnified Parties may not maintain insurance which will cover me for personal injury, property damage or medical expenses, and I accept full responsibility for the costs of treatment for any injury or damages suffered by me while participating as a volunteer in connection with the Championship. I have fully informed myself of the contents of this release by reading it before signing it and agree to be bound by the terms set forth herein in consideration for performing volunteer services.
WAIVER: By finalizing and processing the completed application, I acknowledge that the USGA may undertake a check of my background. I understand and agree that I will sign any additional paperwork in order for the USGA to undertake such a background check. I also confirm and understanding that my volunteer position is not guaranteed, nor if granted, guaranteed for any length of time and that the USGA may deny or end my volunteer opportunity at any time, for any reason, with or without prior notice. I also give the USGA and the Championship the right to use my likeness in archival or advertising print media and/or footage. The USGA, the Championship, Oakmont Country Club and Longue Vue Country Club do not discriminate in the selection among volunteer applications on the basis of race, color, creed, age, national origin, religion, or disability.
